SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Screen, assess, diagnose, creat treatment plans and conduct therapeutic interventions with clients
* Maintain regular, ongoing contact with clients, caregivers and physicians.
* Conduct reassessments and monitor clients' progress.
* Complete and submit timely paperwork and report.
* Review and modify care plans as directed by the client/caregiver during the reassessment and obtain the clients signature of approval.
* Maintain regular, ongoing contact with clients, caregivers, physicians, and service providers listed on service plan through home visits and phone calls.
CLIENT'S REQUIRED EXPERIENCE & EDUCATION FOR CASE MANAGER
* Must be a licensed in the state of Michigan (LPC, LMSW, LLMSW, LP, LLPC, LLP).
