SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
The Supports Coordinator must adhere to Person Centered Planning principles, the Michigan Mental Health Code, and professional standards and code of ethics in the delivery of services/supports to persons served.
* Provide case management services to clients as assigned as part of response to mental health related 911 calls
* Identify community members in crisis and offer mental health services options
CLIENT'S REQUIRED EXPERIENCE & EDUCATION FOR CASE MANAGER
* Must be a licensed in Michigan (LMSW, LLMSW, LPC, LP, LLP).
