Our client is seeking a Case Manager to provide care and service to Brooklyn residents.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Individual & Family counseling, home visits, school visits, monitoring of medical, mental health, & educational services
* Carry out a variety of casework activities within the child welfare system, with supportive services and written documentation as required.
* Maintain a case load of 12 families
* Be trained & implement a Solution Based Casework model approach
* Conduct face to face contacts (home visits/office visits) as specified in the service plan(s)
* Provide assistance in accessing primary medical care, assistance with adherence to medication regiments, nutritional services, and assessments.
* Provide crisis intervention to clients and their families as needed
* Maintain regular personal contact, developing a trusting working relationship with clients
REQUIRED EDUCATION AND EXPERIENCE
* Minimum of a Bachelors Degree in Social Work or rlated field
* Minimum of three year experience in child welfare or other social services, including day car, youth services orother related programs that involve direct work with children and families
* Good verbal and written communication skills
* Mudt be computer literate
* Spanish, Mandarin, Cantonese, Arabic, or Polish speaking strongly preferred
