One of our clients is seeking Bachelor level Case Managers for community outreach or partial care setting in Mercer and Middlesex County.
HOURS
* Monday through Friday
* 8:30am-5:00pm with a hr lunch break
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Maintaining client records
* Helping with individual recovery plans
* Assist in activities which will increase their independence
* Community based services
* Facilitate service linkage
CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Minimum of two (2) years of experience working with adults with serious mental illness
* Minimum of a Bachelors Degree in Human Services Field
* CPR
* TB/Skin Test within the past year
* Personal vehicle and reliable transportation
* Valid drivers License and "clean" DMV check
