One of our clients is seeking a bilingual case manager to provide service in Philadelphia.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide support that will enhance individuals' quality of life through connection with community resources and treatment planning services.
* Provide personal care management to individuals with Mental Health and other intellectual and developmental disabilities.
* Complete documentation and crisis management services when needed.
REQUIRED SKILLS AND EXPERIENCE
* Minimum of a Bachelors Degree
* Minimum of one (1) year experience working with persons with intellectual, developmental disabilities or case management
* Ability to complete and pass clearances: FBI, Child Abuse (ACT 126), and Criminal
* Must be able to produce current medical/health documents including: physical examination, Tuberculosis/TB Skin Test, CPR, and First Aid certification
* Experience in community based residential programs (preferred)
* Valid Driver's License
* Persons with experience in the following roles are welcome to apply: Case manager, Care Manager, BHT, MHT supervisor.
