Our client has 5 opportunities that involve performing Case Management for clients who are residents of Residential Homes in the Northford area! You will apply your Human Services skills in supporting adults and youth with mental health, addiction, homelessness and/or behavioral issues.
You will work with several of their residents on initial assessments, recommending & arranging supportive services, performing reassessments and performing life skills training programs.
You will also enter your case notes into their systems on a timely basis.
Available schedules include the following:
* Sun - Thurs 7 am - 3 pm
* Tues - Sat 7 am - 3 pm
* Sun - Thurs 3 pm - 11 pm
* Sun - Thurs 3 pm - 11 pm
* Sat & Sun 7 am - 3 pm
Candidates need to have a BA/BS degree, some Human Services or Case Management related experience and a compassionate, patient and understanding approach to others.
